چکیده
I n t e r v e n t i o n Patients were allocated to endoscopy plus the noninvasive 14C urea breath test (n = 352) or the breath test alone (n = 356) for determination of H. pylori status. Patients were informed of their status after the test, and patients with positive results were prescribed a 7-day course of H. pylori eradication treatment with omeprazole, 20 mg twice daily; clarithromycin, 250 mg 3 times daily; and amoxicillin, 500 mg (or metronidazole, 400 mg) 3 times daily.

OBJECTIVE To compare the efficacy of non-invasive testing for Helicobacter pylori with that of endoscopy (plus H pylori testing) in the management of patients referred for endoscopic investigation of upper gastrointestinal symptoms. DESIGN Randomised controlled trial with follow up at 12 months. SETTING Hospital gastroenterology unit. PARTICIPANTS 708 patients aged under 55 referred for e...
متن کاملHelicobacter pylori "test and treat" or endoscopy for managing dyspepsia: an individual patient data meta-analysis.
BACKGROUND & AIMS Helicobacter pylori "test and treat" has been recommended for the management of young dyspeptic patients without alarm symptoms, and trials have suggested that it is as effective as endoscopy. However, none of these trials have had sufficient sample size to confirm that "test and treat" costs less or to detect small differences in effect. A collaborative group has prospectivel...
متن کاملIs repeat testing needed for Helicobacter pylori?
Helicobacter pylori is a major cause of gastritis and gastric and duodenal ulcer disease, and it increases the risk of gastric cancer. Diagnosis and treatment of H pylori infection has become an essential part of the evaluation of patients with dyspepsia and other upper abdominal symptoms. Until recently, diagnosis was made primarily by endoscopic biopsy, and treatment was initiated primarily b...
متن کاملNoninvasive Helicobacter pylori testing for the "test-and-treat" strategy: a decision analysis to assess the effect of past infection on test choice.
BACKGROUND Clinical guidelines support a noninvasive Helicobacter pylori "test-and-treat" strategy for individuals with uncomplicated dyspepsia. However, consensus is lacking regarding the preferred noninvasive testing method. OBJECTIVE To use decision analytic modeling to estimate the clinical and economic outcomes associated with noninvasive tests designed to detect either H pylori antibody...
متن کاملCost-effectiveness of six strategies for Helicobacter pylori diagnosis and management in uninvestigated dyspepsia assuming a high resource intensity practice pattern
BACKGROUND Initial assessment of dyspepsia often includes noninvasive testing for Helicobacter pylori infection. Commercially available tests vary widely in cost and accuracy. Although there is extensive literature on the cost-effectiveness of H. pylori treatment, there is little information comparing the cost-effectiveness of various currently used, noninvasive testing strategies. METHODS A ...
